Stenosing ligamentitis - Knott's disease

Stenotic ligamentitis or “trigger finger” can be caused by prolonged, hard and monotonous work or short-term unusual overload in the wrist. This disease is also called Knott’s disease.

Ligamentitis symptoms are painful fingers, a movable thickening forms at their base above the tendons, which interferes with bending and flexing the fingers. Fingers are especially painful and swollen in the morning, when the fingers “bounce” while bending and unbending them. As the ligamentitis progresses, it becomes difficult to bend or unbend the fingers. Initially, splints or steroid injections may help in the treatment of the ligamentitis, but if these do not help, surgery is required. During it, with a small incision in the skin of the wrist the tunnel is enlarged, which allows the tendon to slide freely.

Knott’s disease or ligamentitis has the following symptoms: pain in fingers, at the base of the fingers, above the tendons, a moving nodule forms, it is difficult to bend fingers.

Splints or steroid injections, as well as finger tendon stretching exercises, may help in the initial treatment of stenotic ligamentitis.

If the above does not work or repeated injections of steroids are required within 1 year, wrist surgery is required. During it, with a small incision in the skin of the wrist the tunnel is enlarged, which allows the tendon to slide freely.

The duration of the operation is on average 30 minutes.

After the suregry, local discomfort and pain should be taken into account, which are well controlled with analgesics. Wounds heal in an average of 2 weeks. The patient needs to change dressings 2-3 times a week. The sutires are removed after two weeks. In the post-surgery period, occupational therapy and the development of movements of the fingers will be required to regain full range of motion in the palm and fingers.

Are steroid injections and surgery the only treatments for stenotic ligament?

Initially, analgesics, ice applications, splint immobilization, and stretching exercises, as well as physiotherapy – magnets, ultrasounds, and others – can be used effectively in therapy. However, keep in mind that if symptoms do not go away within 2-3 months, a steroid injection may be the most effective remedy.

How often can a steroid injection be given?

In the case of stenotic ligament, more than 3 injections at least 6 weeks apart are not recommended. However, if the injection does not work, it is better to opt for an operation.
